Overview

Assesses needs and coordinates services for elderly clients through grant programs.

Must successfully pass an AHCA Level 2 screening for this position

Qualifications

Minimum Education and Experience Requirements: Bachelor's degree in health care services, Social Service, Gerontology or related field. Two years of experience in social service or community health environment working with the elderly or other related field preferred. Familiarity with community health, case management, and community resources preferred. One year of staff supervisory experience in a social services or health care services setting, and a minimum of 1 year of experience providing services to persons who have dementia preferred. Basic computer skills required. Strong organizational skills, good documentation skills, and problem-solving ability required. Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a frequent driver Licensure/Certification/Registration: Must complete Case Management certification by the Department of Elder Affairs within three to six months of hire. Valid Florida driver's license and auto insurance required. Current CPR Heart Saver card.
